Frequently Asked Questions

1. What insurances do you accept?
          There are so many plans now, it is best if you contact your insurance customer service and ask them if we are an in-network provider.

2. Can I get my free Wellness Exam?
          Yes. Please be very aware that the definition of this exam is: A periodic comprehensive preventive medicine evaluation to include age      and gender appropriate history, anticipatory guidance, risk factor reduction interventions, and the ordering of preventative laboratory or diagnostic procedures. This does not include the discussion or treatment of new or ongoing health problems or medication refills.

3. What about managed Care and Referrals?
          If your insurance requires referrals, you will need to CONTACT THE SPECIALISTS OFFICE. That office will generate a fax to our office containing pertinent procedure codes, diagnosis codes, provider ID numbers, and number of visits requested. Without this information we cannot request the referral from your insurance. Please allow us at least one week in advance to obtain your referral. NO retroactive referrals will be given.

4. What forms of payment do you accept?
          We accept cash, local checks, debit cards, Visa and Mastercard. Payment is expected at the time of your visit.

5. What if I don’t have my insurance card yet?
          You must have the card with you at the visit, we cannot verify your benefits without it.

6. What if there is a different Doctor’s name on my insurance card?
          Contact you insurance customer service and have the name changed to Dr. Strikowsky. Typically, these changes are not effective until the beginning of the following month.
